TERMS & CONDITIONS

The normal treatment is 20 minutes for $20. You may request a completely filled out 'superbill' to bill your insurance. If you are paying with a check, please add $5. We need 24 concellation notice or a $10 fee will be charged. You may request an initial consult at no cost. Also, feel free to call or e-mail the doctor if you any questions. Thank You!
